IN-HOME CASE MANAGEMENT

WHAT IS IT?

Our Home-Based Initiatives is a in-home case management program that supports families with children of all ages. Family Support Specialists are professionally trained individuals who can serve as a mentor to families that seek assistance in dealing with life’s daily challenges.

CAPS Home-Based Initiatives is the local provider for the Community Partners for Safe Families program.

WHAT CAN A FAMILY SUPPORT SPECIALIST DO TO HELP?

Family Support Specialists help you build self-confidence, self-sufficiency, and coping skills. Many parents benefit by understanding child development while learning how to improve their home environment.

Family Support Specialists assist in making social connections and using community resources so families are not alone in the parenting process. Family Support Specialists provide support and encouragement, by goal setting and practicing helpful parenting techniques, they focus on what is going well and address the needs of families by referring to community agencies when available.

 

 

WHO CAN RECEIVE IN-HOME CASE MANAGEMENT?

• Parents of a child under 18

• Residents of Elkhart County

• Those interested in learning more about parenting

• Those who would like a mentor to visit them in their home
