HOME-BASED INITIATIVES
Program Description
In partnership with SCAN, we offer Community Partners services as support for families with children ages 17 or younger living in the home. We can also serve families when they are prenatal or if they have an 18 year old still in school. This program, administered by SCAN, funded by the Indiana Department of Child and Family Services, provides assessments and home-based case management services to connect families to resources to strengthen the family. Families work with their case managers on mutually established goals to move the individuals and family unit towards economic and family stability. Through this program, families can access services such as:
– Housing crisis assistance
– Financial education and planning
– Home scheduling assistance
– Navigation of a medical diagnosis
– Job Search
– And so much more!
WHO CAN RECEIVE IN-HOME CASE MANAGEMENT?
• Parents of a child under 18
• Residents of Elkhart County
• Those interested in learning more about parenting
• Those who would like a mentor to visit them in their home
